Our specialist Care Team receives your enquiry. We will complete an initial assessment so that we can understand your care and support needs.

We will arrange for a Cherry Sprouts Care Assessor to visit you at home or college and undertake a care and risk assessment in order to capture your background, learning style, care needs and together we will create an individual independence plan.

We will finalise your care plan with you and your family, commissioning team, education provider or work placement and then identify the most suited care package and support worker for you. We expect you to take the lead in creating your support, health and wellbeing plan.

We will work with you to ensure you have a smooth transition to our services of support and care.
